The OFI Supervisor in Cook wanted a weekly report to monitor the progress of TANF customers. The CRS developed a form on an EXCEL spreadsheet. The Supervisor made a few changes and this form is being used in Cook County on a Weekly basis. The ES worker will review with OFI Supervisor each week.

The form has Name, Client ID, Activity /Hours( 2 columns), How hours are Monitored or Verified, Date and Place for comments/actions taken.
