The ES Case Managers refer 5
of their problem cases to the CRS to track on a weekly basis. There are a variety of reasons that the cases
are referred to me to track. Some of
these cases are community service disabled participants or those claiming medical
disabilities that are required to complete work assignments at least twice a
week to keep them engaged. All mandatory
GED and transitional school clients are referred for weekly monitoring of their
attendance. The tracking form is an
important tool to track daily attendance of WEX clients. The tracking forms are
also used when case managers need medical conditions verified as well as when a
criminal history needs to be conducted with local law enforcement agencies. In
order to make sure that the home and site visits were documented in a format
that would help the ES Case Managers, Joe Mullis, CRS, came up with the
attached tracking form. This form has
been beneficial to the case managers in monitoring these cases. The tracking form gives a detailed account of
all the contacts with the client as well as all the special accommodations that
are made on their behalf. The tracking forms are maintained by the CRS and
updated after every contact then emailed to the Case Managers. The client tracking
form has also been used to provide vital information at fair hearings. The
tracking form provides a detailed account of all the contacts and
accommodations that may have been made for the clients with disabilities or
other clients who have failed to participate.
We have been successful in winning fair hearings due in large part to
the documentation on the tracking form.
